MR-guided Focused Ultrasound at KIMS — First in Telangana/AP
KIMS Neuroscience, Hyderabad performed the first MR-guided Focused Ultrasound (MRgFUS) procedure in Telangana and Andhra Pradesh. MRgFUS offers patients with essential tremor and Parkinson's disease tremor an FDA-approved, non-invasive neurosurgical treatment — with no incision, no anaesthesia, no radiation, and no implanted device — that is completed in a single session. Patients experience tremor improvement during the procedure itself, while lying awake in the MRI scanner.
What MRgFUS Is
MRgFUS is a non-invasive neurosurgical treatment that uses 1,024 focused ultrasound beams, guided in real time by MRI thermometry, to heat and precisely destroy a small target deep within the brain — without any incision, implant, anaesthesia, or radiation. For tremor disorders, the target is the ventral intermediate (Vim) nucleus of the thalamus, which is part of the tremor circuit. The patient is awake throughout. Tremor typically improves or resolves during the procedure itself.
- What MRgFUS is — one sentence
- MRgFUS (MR-guided Focused Ultrasound) is a non-invasive neurosurgical treatment that uses 1,024 focused ultrasound beams, guided in real time by MRI thermometry, to create a precise thermal lesion in the Vim nucleus of the thalamus — without any incision, implant, anaesthesia, or radiation — that interrupts the abnormal tremor circuit, typically reducing tremor by ~75% in eligible patients (NEJM 2016, 12-month data) on the treatment day itself.
- Who it is for — the three requirements
- Three conditions must all be met: (1) Diagnosis: essential tremor (ET) of the dominant hand/arm, or tremor-dominant Parkinson's disease. (2) Medical therapy failure: at least two appropriate medications tried at adequate doses — propranolol + primidone for ET; levodopa ± dopamine agonist for PD — with inadequate tremor control or intolerable side effects. (3) Technical eligibility: skull density ratio (SDR) ≥ 0.4 on head CT. Approximately 20–30% of clinically eligible patients fail the SDR criterion.
- How it compares to DBS
- MRgFUS: no surgery, no implant, no anaesthesia, no battery replacement, non-adjustable (irreversible lesion), unilateral only, same-day procedure and discharge. DBS: open surgery, implanted electrodes and battery, general anaesthesia, fully adjustable, can be bilateral, requires battery replacement every 5–9 years. The choice is guided by tremor dominance (MRgFUS for a unilateral tremor-dominant presentation) versus the need for adjustability or bilateral treatment (DBS).
- What the outcome looks like
- From the NEJM 2016 FDA registration trial: 75% mean tremor reduction at 12 months; 80–85% of patients achieve clinically meaningful improvement; 62% improvement in disability scale; 10–20% partial tremor recurrence at 5 years; <5% permanent side effects. Most patients leave the treatment table with tremor that is substantially reduced or absent.
- What it treats and what it does not
- MRgFUS treats essential tremor of the dominant hand/arm (FDA 2016) and tremor-dominant Parkinson's disease (FDA 2018). It does NOT treat Parkinson's disease where bradykinesia and rigidity dominate, MS tremor, cerebellar ataxia-related tremor, drug-induced tremor, dystonic tremor, or psychogenic tremor. For bilateral tremor, bilateral thalamotomy carries increased dysarthria and swallowing risk — DBS is preferred.
- What makes KIMS the right place in South India
- KIMS performed the first MRgFUS procedure in Telangana and Andhra Pradesh, using the Insightec Exablate Neuro 1,024-transducer system — the same FDA-registered device used in the NEJM trials. Before KIMS, patients across South India had to travel to Delhi, Mumbai, or abroad. The KIMS movement disorders team assesses every patient for MRgFUS versus DBS. Out-of-Hyderabad patients can complete eligibility assessment and treatment in a combined 2-day visit. Insurance pre-authorisation and Patient Assistance Programme support are available.

MRgFUS in Hyderabad — KIMS Neuroscience
Imagine treating a debilitating tremor that has made it impossible to eat with a spoon, write your name, or button a shirt — without any surgery, any incision, any anaesthetic, any implant, and any radiation. Imagine lying awake in an MRI scanner for a few hours and leaving the same day, with a tremor that has largely disappeared. This is what MR-guided Focused Ultrasound offers to eligible patients with essential tremor and Parkinson's disease tremor.
MRgFUS uses 1,024 ultrasound transducers arranged around a specially designed helmet to focus converging ultrasound waves at a precisely defined target in the thalamus. Real-time MRI thermometry monitors the temperature at the target throughout the procedure, ensuring that the heating is contained to the planned target and that adjacent structures are protected. When the target temperature reaches 55–60°C, a small focal lesion is created — interrupting the abnormal tremor circuit. The tremor stops, or is dramatically reduced, while the patient is still on the treatment table.
At KIMS Neuroscience, Hyderabad, the first MRgFUS procedure in Telangana and Andhra Pradesh was performed — bringing a technology that had previously been available only in a handful of Indian centres to South India. For patients across Telangana, AP, and the surrounding states who have been unable to control their tremor with medication and are looking for an alternative to deep brain stimulation surgery, MRgFUS at KIMS represents the closest thing to a surgical cure without surgery.
